Waiver and Release From Liability

This agreement releases valuable rights. Please read it carefully and completely. Despite all reasonable precautions taken by the league, injuries can occur; and even the best equipment cannot always prevent injuries. We (I) being the Parents or Legal Guardians (“Releasors”) of the above named player, do hereby give consent for this child to participate as a player in the Falcon Football Team. We (I) on behalf of any person that may have a claim as a result of the player’s death, injury, disability, or loss of personal belongings hereby voluntarily waive and FOREVER RELEASE, DISCHARGE AND COVENANT NOT TO SUE the St. Philip's Parish, it’s employees, insurers, coaches, volunteers and Falcon Football personnel, from any and all liabilities, claims, demands, or cause of action that we (I) may hereafter have from injuries of damages arising out of the player’s participation in any activities of Falcon Football, Inc., including but not limited to losses caused by passive or active negligence of the releases.

Releasor understands and acknowledges that the activities of tackle football have inherent dangers and cannot be eliminated. RELEASOR EXPRESSLY AND VOLUNTARILY ASSUMES ALL RISK FOR PERSONAL INJURY, TEMPORARY OR PERMANENT DISABILITY, OR DEATH sustained while participating in Falcon Football/including the risk of passive or active negligence of the releases or any other participant.

Releasor acknowledges and fully understands that each participant including this player, will be engaging in activities that involve risk of serious injury, including temporary or permanent disability and death, and that social economic losses may result not only from the player’s own actions, inactions or negligence, but also from the actions, inactions, and negligence of others notwithstanding the rules of play or conditions of the premises and any equipment used. Further, the releasor acknowledges and fully understands that there may be other risks not known or not reasonably foreseeable at the time.

We (I) also understand that sufficient health insurance must be secured covering the player by June 1 2026, in order to allow the player to participate. We (I) accept responsibility for returning or making restitution of any team-owned equipment lost, stolen, or unreasonably damaged as determined by the coaches. We (I) understand that the player fee will not be refunded when a player leaves the program for any reason after the season begins. We (I) authorize any coach(es) to admit the player to the nearest medical facility and/or seek the care of a physician if conditions warrant such action.

A photocopy of this Waiver and Release shall be considered as valid and acceptable as the original.

This agreement is in effect for the following period:
January 1, 2026, through December 31, 2026.
